The new Z83 format was introduced on January 1, 2021 and has been in force ever since. Requests submitted using the previous form will not be considered. Therefore, it is the applicant’s responsibility to ensure that the form used for the application is the new approved form. You can download this form from the Department of Public Service and Government, or you can get a copy from your local authority.
